INTAKE PROCEDURE:Must be referred to SEARCH through coordinated access or The Beacon; see website more information.
DESCRIPTION:Provides intensive case management for chronically homeless clients referred from Coordinated Access or The Beacon, with a disabling condition to include, substance use disorders and/or serious mental illness. Provides assistance in securing medical/mental health care, public benefits, and housing.
