DESCRIPTION:Provides application assistance in the greater Houston area for food stamps, TANF, Medicaid, and the Children's Health Insurance Program (C.H.I.P.), as well as other state benefits programs. Application assistance consists of providing access to the application, verifying the applicant's application is completed correctly, and delivering the application and any additional documents to the Health and Human Services Commission office.

INTAKE PROCEDURE:Walk-ins accepted

DOCUMENTS REQUIRED:Social Security card(s) and valid photo identification for all household members requesting assistance on the application; Proof of all household income received over the last 3 months (paycheck stubs or statement from employer, Social Security income, workers compensation, retirement income, etc.); Proof of all household expenses (rent or mortgage payment, utility bills, medical bills, child care support, loans, etc.); Proof of resources such as vehicle ownership and checking/savings account statements; For those requesting medical assistance: U.S. passport or birth certificate for all requesting assistance
