Disaster Case Management

DESCRIPTION:Provides disaster case management for persons who are recovering from Hurricane Harvey. The program assists individuals in developing a recovery plan which identifies unmet needs, locates services, and mitigates the impact of loss on the household.

INTAKE PROCEDURE:Call for an appointment or can also send an email with contact information for an agency representative to call and set up an appointment; does not accept walk-ins for disaster case management. If there is no answer when calling, leave a message and a disaster case manager will make a return call.


DOCUMENTS REQUIRED:Varies according to need; be prepared with FEMA registration number, insurance claim information, and photos or other documentation of damage
