Disaster Related Case Management

DESCRIPTION:Provides case management for immigrants/refuges and older adults who were affected by the April 2016 flooding. Persons must have an appointment and be willing to go through a full assessment of their needs. Services to meet those needs are provided only after an in-depth review of the person's situation. Some services could include food and prescription assistance.

INTAKE PROCEDURE:Must call phone line, leave a message and someone will call back to schedule an appointment. There are no walk-ins accepted.
